To be eligible for COMMCARE services: Philosophy:
  • Must have a diagnosis of Traumatic Brain Injury (TBI) as the primary diagnosis on the MA-51 formMedical Evaluation
  • Must be between the ages of 21 and 59
  • Pennsylvania Resident
  • Must be Nursing Facility Eligible
  • Must have substantial functional limitations in 3 or more of the following major life activities:
  • Mobility
  • Communication
  • Independent Living
  • Self-Care
  • Behavior
  • Self-Direction
  • Cognitive Capacity
  • Directed and controlled to the maximum extent as possible by the consumer
  • Meet Health and Safety needs of the consumer
  • Conform with Federal and State requirements
  • Be cost effective
  • Currently, Unlimited Care Providers serves 22 counties; Armstrong, Bedford, Blair, Cambria, Cameron, Centre, Clarion, Clearfield, Clinton, Elk, Forest, Fulton, Huntingdon, Indiana, Jefferson, Lycoming, McKean, Potter, Somerset, Tioga, Warren and Westmoreland.
